Revoke Door Access Form

Please use this form to revoke keys and Bulldog Card access to buildings on the Montana Western campus. Once submitted, this form will be forwarded to Facility Services and the Bulldog Card Office.  

For Keys: Please have key holder return keys to the Facility Services Office upon completion of this form anytime Monday-Friday 8 a.m. to 5 p.m. If you have questions, please contact [email protected]. 

For Bulldog Card: the Bulldog Card Office or Facility Services will notify the Access holder when access has been revoked.
